photography unlimited

Should I write a contract for printing rights?
I’m starting out with a little free photography for friends, family, etc and would like to provide a CD with unlimited printing rights. However, I don’t want them to have any trouble with this should they take the CD to Wal-Mart or Sams, etc. I remember I took a CD with my own work and was asked if they were mine and if I had the rights to them… should I write some type of contract to go along with the photos so that they will have no issues?
Should I use my legal name or can I use the name I am hoping to be known by?
Thank you Peter, is there one I can use online? A more generic form?
Should I have them also sign a waiver that they are consenting for me to photograph them?
On each CD your publish, also include a license. This license can be written however you (the author) would like, so to include unlimited rights, limited rights (only printing, no distribution), or whatever you’d like.
